Frozen shoulder (adhesive capsulitis): What to expect from RMT and acupuncture

Frozen shoulder causes progressive pain and stiffness, making everyday tasks like dressing and reaching overhead difficult. It often unfolds in stages (freezing, frozen, thawing) over months to years. Acupuncture and manual therapy (including massage and joint mobilization) can help reduce pain and improve range of motion in the short term, especially when combined with gentle exercise and activity pacing

Typical frozen shoulder symptoms

  • Gradual onset of deep shoulder ache, often worse at night and with reaching behind the back or overhead

  • Marked stiffness: limited active and passive range of motion in multiple directions

  • Common in people aged 40–60 particularly women, and in those with autoimmune conditions, or after shoulder immobilization

Self-care and medical management basics

  • Keep the shoulder moving within comfort: gentle range-of-motion (pendulums, wall walks, cane-assisted external rotation) several times daily

  • Heat before stretching, ice after if sore; avoid long periods with the arm immobilized

  • Corticosteroid injection plus physiotherapy often provides the best short-term relief; manual therapy and exercise add benefit over time

Where massage/manual therapy fits in

Deep transverse friction massage and soft-tissue mobilization can reduce pain and improve shoulder motion in the short term for some people with adhesive capsulitis. Clinical reviews suggest manual therapy works best when combined with joint mobilization and exercise, particularly in the frozen and thawing stages

Consider RMT or manual therapy if you:

  • Have significant muscle guarding around the shoulder and upper back that limits movement

  • Want hands-on work to calm pain so you can tolerate daily mobility exercises.

What to expect:

  • 45–60 minute sessions focusing on shoulder girdle, upper back, and gentle mobilization, plus a home program; often weekly for 6–10 weeks

Where acupuncture fits in

Systematic reviews suggest acupuncture can reduce frozen shoulder pain and improve function and flexion range of motion in the short to mid term, with very low to low certainty evidence. A 2025 network meta-analysis found acupuncture-based therapies (especially combined with mobilization or massage) ranked among the more effective options, with few adverse events

Consider acupuncture if you:

  • Prefer needles over deep pressure or have had good past results with acupuncture

  • Want an adjunct to physio/manual therapy to help with pain and sleep

What to expect:

  • 20–30 minute sessions, 1–2 times/week for 4–6 weeks, using local (shoulder/upper back) and distal points

Red flags: When to see a doctor first

Seek medical care before booking treatment if you have:

  • Fever, feeling unwell, or a hot, swollen shoulder (possible infection)

  • History of cancer, unexplained weight loss, or unremitting night pain

  • Recent significant trauma, obvious deformity, or inability to move the shoulder after an injury

  • Chest pain, shortness of breath, or sweating with shoulder/arm pain (possible heart/lung issue)

How this works at Alaria Therapeutics (Victoria, BC)

At Alaria Therapeutics in downtown Victoria, we assess shoulder mobility, pain patterns, and daily limitations to decide whether RMT, acupuncture, or a combination best supports your exercise and medical plan. Many extended health plans cover RMT and acupuncture; we provide direct billing to most extended health insurers. If you’re unsure where to start, tell us your main symptoms (night pain, stiffness, what movements are hardest) and we’ll suggest a 4–10 week plan with home exercises and progress checks.
